    Optimal Control and Sensitivity Analysis of a Fractional Order TB Model

    A Caputo fractional-order mathematical model for the transmission dynamics of tuberculosis (TB) was recently proposed in [Math. Model. Nat. Phenom. 13 (2018), no. 1, Art. 9]. Here, a sensitivity analysis of that model is done, showing the importance of accuracy of parameter values. A fractional optimal control (FOC) problem is then formulated and solved, with the rate of treatment as the control variable. Finally, a cost-effectiveness analysis is performed to assess the cost and the effectiveness of the control measures during the intervention, showing in which conditions FOC is useful with respect to classical (integer-order) optimal control.Comment: This is a preprint of a paper whose final and definite form is with 'Statistics Opt.     Optimal control of a fractional order epidemic model with application to human respiratory syncytial virus infection

    A human respiratory syncytial virus surveillance system was implemented in Florida in 1999, to support clinical decision-making for prophylaxis of premature newborns. Recently, a local periodic SEIRS mathematical model was proposed in [Stat. Optim. Inf. Comput. 6 (2018), no.1, 139--149] to describe real data collected by Florida's system. In contrast, here we propose a non-local fractional (non-integer) order model. A fractional optimal control problem is then formulated and solved, having treatment as the control.     Parasitoids were recovered from adults and egg masses of Mormidea angustata Stal and Oebalus ypsilon-griseus (De Geer). Three parasitoid species were recovered; the species were identified as Beskia aelops Walker (Diptera:Tachinidae), Gymnoclytia sp.     Con el objetivo de identificar los factores relacionados al abandono del anticonceptivo inyectable mensual en mujeres en edad fértil que acudieron al servicio de planificación familiar del Centro de Salud Collique III Zona en Comas durante unos meses del 2021, se realizó una investigación cuantitativa, del tipo básica, con diseño no experimental, transversal y descriptivo correlacional simple, con una muestra censal de 100 unidades de análisis, quienes contestaron un cuestionario previamente validado y con confiabilidad aceptable, previo consentimiento informado. Los resultados demostraron que las mujeres en estudio abandonaron el anticonceptivo inyectable mensual en un 68%, entre los 4 a 7 meses de uso se caracterizaron por tener entre 18 a 29 años, tener secundaria, ser católica, solteras y convivientes y amas de casa; los motivos de abandono fueron en mayor frecuencia efectos adversos y cambio de método; los efectos adversos más frecuentes fueron cefalea y variación de peso; concluyéndose que los factores socioculturales como edad, estado civil, grado de instrucción, religión y ocupación, así como los factores reproductivos como menarquia e inicio de vida sexual, se relacionaron significativamente con el abandono del anticonceptivo inyectable mensual (p < 0.05); mientras que los factores reproductivos parejas sexuales y paridad, no se relacionan.With the objective of identifying the factors related to the abandonment of monthly injectable contraceptives in women of fertile age who attended the family planning service of the Collique III Zone Health Center in Comas during a few months in 2021, a quantitative, basic, non-experimental, cross-sectional and descriptive correlational simple research was carried out with a census sample of 100 units of analysis, who answered a previously validated questionnaire with acceptable reliability, with prior informed consent. The results showed that 68% of the women in the study abandoned the monthly injectable contraceptive, between 4 and 7 months of use, and were characterized by being between 18 and 29 years old, in high school, Catholic, single and cohabiting, and housewives; the reasons for abandonment were most frequently adverse effects and change of method; The most frequent adverse effects were headache and weight change; it was concluded that sociocultural factors such as age, marital status, education level, religion and occupation, as well as reproductive factors such as menarche and sexual debut, were significantly related to the abandonment of monthly injectable contraceptives (p < 0.05). 05); while reproductive factors such as sexual partners and parity were not related

    Big Health Data and Cardiovascular Diseases: A Challenge for Research, an Opportunity for Clinical Care

    Cardiovascular disease (CVD) accounts for the majority of death and hospitalization, health care expenditures and loss of productivity in developed country. CVD research, thus, plays a key role for improving patients' outcomes as well as for the sustainability of health systems. The increasing costs and complexity of modern medicine along with the fragmentation in healthcare organizations interfere with improving quality care and represent a missed opportunity for research. The advancement in diagnosis, therapy and prognostic evaluation of patients with CVD, indeed, is frustrated by limited data access to selected small patient populations, not standardized nor computable definition of disease and lack of approved relevant patient-centered outcomes. These critical issues results in a deep mismatch between randomized controlled trials and real-world setting, heterogeneity in treatment response and wide inter-individual variation in prognosis. Big data approach combines millions of people's electronic health records (EHR) from different resources and provides a new methodology expanding data collection in three direction: high volume, wide variety and extreme acquisition speed. Large population studies based on EHR holds much promise due to low costs, diminished study participant burden, and reduced selection bias, thus offering an alternative to traditional ascertainment through biomedical screening and tracing processes. By merging and harmonizing large data sets, the researchers aspire to build algorithms that allow targeted and personalized CVD treatments. In current paper, we provide a critical review of big health data for cardiovascular research, focusing on the opportunities of this largely free data analytics and the challenges in its realization
